§ 110-113. Form of the petition.
The petition shall contain the following information:
(1)
A designated individual, including his address and telephone number, who will be or act as the official representative of the petitioners;
(2)
Specific designation of the boundaries of the proposed special assessment district to be benefited by the project;
(3)
Signatures as the names appear on the tax roll with respect to 51 percent or more of the owners of the lands located within the proposed special assessment district or the owners of 51 percent or more, of the lands determined by area located within the proposed special assessment district. The specific legal description as it appears on the tax roll or legal ownership documents of the lots or land shall be set forth opposite the name or names of the signer or signers of the petition;
(4)
Property ownership map delineating actual ownership of each benefited unit within the proposed special assessment district; and
(5)
Failure to supply any of the above information in the required form shall result in the return of the petition to the designated individual referred to in subsection (1) above.
